(4) Fees
: the fee for each request for notification of disclosure of personal information or purpose of use is \700 (tax included). Please include postage stamps or a money order totaling \700 with the documents necessary for your request.
(5) Replies
after having been checked by the company, a written reply to your request will be sent to the address noted on the request form. Please be forewarned that it usually takes approximately 3 weeks for requests to be processed and replies to be sent out.
(6) Regarding non-disclosure of information
in the event that the request sent to us corresponds to any of the below, we may be unable to disclose all or part of the information included in the request. Should this occur, we will send a reason in writing along with notification of that information that cannot be disclosed. Please be forewarned that even should information not be disclosed, fees paid and any postage fees incurred pertaining to requests will not be refunded.
